Phoenix Compliance Shop is a registered trade name of Phoenix Laboratory Consulting, LLC, a Kentucky limited liability company. Orders are processed by FastSpring, our authorized reseller and Merchant of Record, which appears on your statement.
Delivery
Electronic downloads are available immediately after payment clears. The manual is delivered as a PDF stamped with the purchaser's name and order reference; the workbook and forms library are delivered as separate .zip files.

License scope
Each purchase authorizes use at one laboratory facility, identified by its CLIA license number or NYSDOH Permanent Facility Identifier (PFI). Multi-site systems, parent corporations, and affiliated laboratories require separate licenses. Contact us for a multi-site quote.
Refunds

What is not included
No consulting, no advice, no review of your validation data or submission packets, and no guarantee of any regulatory outcome. We will fix a failed order, a license key that does not work, or an export that will not generate — that is the limit of what is included.
